Requirements

• Current Official Certificate of Registration / Enrollment signed by Registrar in wet signature (Senior High School Diploma or Proof of College Admission may be submitted if already graduated in SHS or currently in Grade 12)
• Latest Copy of Grades signed by Registrar (wet signature)
• Proof of Income (any of the following):
• Parents' or guardians' Recent Income Tax Return
• BIR Tax Exemption Certificate
• Parents' Certificate of Indigency
• Recent Employment Contract or Proof of Income (for children of OFWs and seafarers)
• Latest copy of electric or water bill (if available)
